Notice of privacy practices

This notice describes how medical information about you may be used and disclosed, how you can access that information, and the privacy rights available to you. Please review it carefully.

Your medical information is collected on the platform of our clinical technology partner, FUSE Health, and received by Beluga Health, PA, the licensed telehealth practice whose providers review your intake. Both are required to protect it under HIPAA and applicable state law.

FUSE Health operates under a signed Business Associate Agreement (BAA), which legally requires them to safeguard your protected health information and use it only as permitted by HIPAA.

This website does not collect or store your medical intake information.

How your health information may be used or disclosed

Your protected health information may be used or disclosed for treatment, including evaluating your medical intake, coordinating care, prescribing when medically appropriate, and sending a prescription to a pharmacy; for payment activities; and for healthcare operations such as quality assessment, compliance, and administration.

Information may also be used or disclosed when permitted or required by law, including for public-health activities, health-oversight activities, judicial or administrative proceedings, law enforcement, preventing a serious threat to health or safety, workers’ compensation, and other legally authorized purposes.

Other uses or disclosures not described in this notice generally require your written authorization. You may revoke an authorization in writing, except to the extent action has already been taken in reliance on it.

Your rights

Subject to applicable law, you may request access to or a copy of your medical record, ask for a correction, request restrictions on certain uses or disclosures, request confidential communications by an alternative method or at an alternative location, and request an accounting of certain disclosures.

You may request a paper copy of this notice at any time. You may also file a privacy complaint without retaliation.

To exercise a right concerning your medical record, contact care@timelesshealth4u.com. We will direct your request to Beluga Health, PA, FUSE Health, or the appropriate healthcare partner that maintains the information.
